This position provides community-based case management services to adults

age 50 and older living within the Pueblo of Laguna Village boundaries.  

A case manager at K’awaika senior center coordinates care, assesses client needs, and connects older adults with essential services to enhance their quality of life. The case manager role at K’awaika senior center is pivotal in enhancing the well-being and independence of older adults by providing comprehensive support, care coordination, and advocacy. Case managers at K’awaika senior centers may also assist with financial counseling, including helping clients apply for Medicaid, insurance coverage, or other assistance programs. They will serve as a primary point of contact for seniors, ensuring continuity of care and advocating for their needs within the community. The role often requires empathy, problem-solving abilities, and the capacity to handle sensitive situations while maintaining professional boundaries.

Key Responsibilities:

A case manager at K’awaika senior center is responsible for assessing the needs of older adults, developing individualized care plans, and coordinating access to services such as healthcare, housing, counseling, and social programs. The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

  • Coordinate all aspects of case management, provide information and

assistance always acting as the client’s advocate.

  • Possess knowledge related to nutrition, mental and physical health, safety,
  • communication, economic resources, activities of daily living, socialization,

and support systems.

  • Work with those experiencing financial, food or housing insecurity.
  • Ability to work with culturally diverse individuals and groups.
  • Assess clients’ needs and maintain on-going contact with clients through home visits, office appointments and phone contacts.
  • Develop client centered service plans with the input of the client.
  • Arrange, coordinate and monitor services at the level required for the client.
  • Maintain client files with accurate up-to-date intake and assessment forms

and case notes.

  • Complete home-delivered meal eligibility assessments and annual

reassessments. Monitor and coordinate client home delivered meals.

  • Develops narrative reports, revisions to plans, case management assessment program forms and Quarterly Client Centered Case Management Report.
  • Coordinating healthcare providers, social service agencies, and community resources to facilitate access to services.
  • Maintaining accurate records and documentation of client interactions, progress, and service utilization.
  • Providing guidance, support, and advocacy for seniors and their families to navigate complex systems such as Medicaid, insurance, or financial assistance programs.
  • Supervising or mentoring junior case managers or interns, depending on the seniority of the role.
  • Follow the K’awaika Senior Center protocol for Home Care Program procedures,

complete necessary forms, coordinate, and monitor services.

  • Make referrals to the appropriate Pueblo of Laguna Community resources and/or external resources.
  • Receive and/or make referrals to Adult Protective Services and collaborate

on cases.

  • Ability to build effective working relationships with Pueblo of Laguna Public Safety.
  • Collaborate with other professionals including physicians, mental health

counselors, home health agencies, hospice, hospital, and nursing

home social workers.

  • Attend Area Agency on Aging Case Manager trainings including Ethics &

Boundaries, Medicare Refresher and Medicare Part D trainings.

  • Actively participate on committees and in organizations that advocate for

older adults, through OIEA

  • Adhere to the American Case Management Association.
  • Exercise professional judgement, maintain confidentiality and treat all clients

with dignity and respect.

  • Ability to build effective working relationships with other Village Officials, volunteers and other community groups.
  • Facilitate a support group at the Senior Center, such as Caregiver Support,

Parkinson’s Support or Low Vision Support.

  • Assist with Senior Center programs as needed. Includes all other possible duties assigned.
  • Represent the Senior Center at professional and community events.

Required Skills and Qualifications:

  • Successful case managers typically possess a combination of education, experience, and interpersonal skills:
  • Education: Bachelor’s degree in social work, psychology, gerontology, Nursing or a related field; a master’s degree is often preferred.
  • Experience: 5 years or more of case management experience, ideally with older adults or in healthcare/social services; supervisory experience is a plus.
  • Certifications: Credentials such as Certified Case Manager (CCM) or Accredited Case Manager (ACM), within 1 year of employment.
  • Skills: Strong communication, organizational, and time management skills; proficiency in case management software and electronic health records; ability to work collaboratively with multidisciplinary teams.
  • Experience working with older adults.
  • Valid New Mexico Driver’s License.
  • Must be able to obtain training and certification for First Aid, CPR and AED operation.
  • Must complete I.H.S food handlers course.

  • Possess specific knowledge of human services for older adults.
  • Knowledge of current human service philosophies and approaches such as

self-determination, person-centered planning and community inclusion.

  • Ability to prioritize, calmly manage and resolve urgent or crisis situations.
  • Ability to multitask and adapt to changing work priorities as needed.
  • Knowledge of the aging process.
  • Ability to work independently and seek out consultation from peers and

others in the aging field when needed.

  • Proficient computer use including knowledge of Microsoft Word, Excel and

Microsoft Publisher.

  • Ability to communicate effectively both orally and in writing.
  • Excellent organizational skills.

Physical Demands:

  • The physical demands here are representative of those that must be met by an

               employee to successfully perform the essential functions of this job.

  • Employee will spend an extensive amount of time on the computer and phone, for charting purposes.
  • Employee will be driving 4-6 hours per day to conduct home visits.
  • The employee may have to lift (10 lbs. or more) and use equipment such as walkers and wheelchairs.

In order to set up services and an evaluation the case manager will do in home

visits. Travel is required for training, meetings and other events. Evening and weekend work may be required for special programs, projects and groups.
